Eligibility criteria at a glance:
- you must be the owner of the pet, over 18 years of age and a resident of the United Kingdom
- your pet mustn't be less than eight weeks old
- your pet mustn't fall under the restrictions of the Dangerous Dogs Act 1991, the Dogs (Northern Ireland) Order 1983, Dogs (Northern Ireland) Act 1991 or any changes to those laws
- you'll need to have all your pet's details to hand, including their medical history
