When my phone was lost it was reported within a couple of hours to the police and T-Mobile who sold me the phone and the insurance.
Due to holidays etc I did not phone the insurance company to ask for the replacement until after the 48 hour deadline and was therefore refused a replacement. Further correspondence from Fonesafe arrived on T-Mobile headed paper, to my mind this makes them part of the same company but this made no difference to my claim.
A very unsatisfied ex customer.
