My elderly parents took out travel insurance with M and S and my father suffered a stroke in southern France, the process to get him home was so inefficient that I ended up driving 600 miles down to the Dordogne and taking a week off work to be with them and to sort the whole business out.
You have to deal with Axa in Germany who once you can convince them that indeed they are liable for the cover are incapable of returning phone calls, making contact with hospitals or GP`s, their travel bookings close at 1900 leading to long delays.
There was one operator in the call centre who would actually do anything - the rest just fobbed me off, goodness knows what my mobile phone bill will be.
Then finally having got to them to bring my Parents home to this country and given them the name and address of the hospital that I had arranged my father to be taken to they tried to take him home.
